Biometric Screening Instructions

RETURN YOUR SCREENING RESULTS BY DECEMBER 1, 2025

You have three options for supplying your results:

  1. If you have already had an annual exam on or after December 1, 2024, and before December 1, 2025, download a physician screening form after registering with eHealthScreenings and ask your doctor to complete it with your biometric information. Submit the completed form to eHealthScreenings.
  2. If you haven’t had an annual exam with biometric screening, it’s time to call your doctor and schedule a visit. It’s important to do this as soon as possible, because your doctor will need to complete the eHealthScreening physician screening form so you can submit it to eHealthScreenings for you to be eligible.
  3. If you can’t get to the doctor, you can visit an approved lab to have your biometric screening. Download a lab requisition voucher after registering with eHealthScreenings and contact eHealthScreenings to schedule an appointment at a participating lab near you.

To register with eHealthScreenings and download the forms:

  1. Go to https://www.ehealthscreenings.com/signup.
  2. If you have participated in a previous screening facilitated by eHealthScreenings, enter your username and password and click Submit. On the following screen under Step 1, enter the Screening Key: LOC372 and Click to Select.
    If you have not participated in screening, under the NOT YET REGISTERED? box, enter the Screening key: LOC372 and click Submit. Enter your assigned default username and password.

    USERNAME: First Initial + Last Initial + DOB (MMDDYYYY) (example: John Smith born 4/23/1975 is JS04231975)
    PASSWORD: Last Name + Last 4 SSN (example: John Smith with social 123456789 is Smith6789)
    *Username and Password are case sensitive
  3. Update account information and click Submit.
  4. Choose the offsite lab or physician screening option. To find an offsite lab, enter your address in the Find Labs by Address section and click Find By Address or click Using my Current Location. Select a convenient lab location by clicking Select next to the lab location.
  5. Acknowledge the Health Screening Consent by checking the "I agree" box at the bottom of the consent. Enter first and last name in the Signature section. Click Continue.
  6. Once you have completed the sign-up process, a confirmation page will be generated for you to print for your records. You will also receive a confirmation email with a lab requisition or physician screening form and instructions on how to complete your screening.

Tests include: total cholesterol, HDL, LDL, triglycerides, glucose, nicotine, blood pressure, height, weight, waist circumference, and body mass index.

REMEMBER: Fasting (no food) is recommended for 9 hours before your appointment. Please drink plenty of water. Black coffee is permitted. Continue to take any prescription medications. If you are diabetic, please consult your physician before fasting.
